About the Role:

RespectAdultDayHealthCare LLC in Winchester, MA is looking for a compassionate and dedicated Part-Time Social Worker with LCSW or LISW license to join our caring team. Responsibilities:

• Conduct comprehensive psychosocial assessments for adult day health care participants
• Develop and implement individualized care plans in collaboration with interdisciplinary team members
• Provide counseling, crisis intervention, and emotional support to participants and their families
• Connect clients with community resources, social services, and support programs
• Monitor and document participant progress, needs, and changes in condition
• Advocate for participants' rights, dignity, and overall well-being
• Facilitate family meetings and coordinate discharge or transition planning as needed

Requirements:

• Bachelor's or Master's degree in Social Work (BSW or MSW) from an accredited program
• Current Massachusetts social work licensure (LCSW, LICSW, or LSW preferred)
• Experience working with elderly, disabled, or medically complex adult populations
• Strong knowledge of community resources, elder care services, and social service systems
• Excellent communication, documentation, and case management skills
• Compassionate, patient, and culturally sensitive approach to client care
• Ability to work collaboratively within a multidisciplinary healthcare team
