Responsibilities
- Develop, implement, and evaluate health plan care management program lines to improve quality and cost-effectiveness.
- Promote continuity of care and support whole-person in-patient and out-patient management, care integration, and care planning.
- Coordinate case management activities, utilization review, and care coordination across interdisciplinary teams.
- Assess patient and member needs, contribute to comprehensive care plans, and monitor progress and adherence.
- Communicate plan of care, interventions, and outcomes with patients, families, and care providers; document interventions accurately.

Requirements
- Active RN license in California.
- Current Basic Life Support (BLS) certification.
- Experience in case management or health plan care management preferred.
- Education: Nursing degree (ADN or BSN).
- Ability to work in a hospital setting and collaborate with cross-functional teams.
